1. Make sure you are registered with PasigPass and your Pasig Health Monitor is updated.

Profilers will validate the answers to online profiling forms before updating your Pasig Health Monitor. NOTE: make sure that the contact number to be put in your online profiling form is correct so that the profilers can be contacted.

2. Will receive SMS advisory regarding the schedule and venue of vaccination

To ensure social distancing and avoid long queuing at vaccination sites, walk-ins are not allowed. Only those who received SMS advisory will be vaccinated.

3. Vaccination Proper

* Valid ID
